In the Primal Rage universe, a meteor shower hits Earth again; technology ceased to exist, civilization fell into ruins, and humanity regressed into tribes, like the Stone Age. A new evolution is taking place in this new post-apocalyptic world, named "Urs" due to radioactive emissions.

Of all the creatures that have appeared, seven stand out, fighting for control of the new world. They are divided into those who want to create peace on Urs, and those who want to throw the world into chaos. Each of them has unique supernatural abilities, meaning any elements of nature, such as life and death, fire and ice. They are considered gods of their element. There are 4 Virtue Gods and 3 Destroyer Gods in total.

Primal Rage is a traditional 2D fighting game in which 2 players choose their own fighter and fight one on one, or one player plays a campaign against a group of opponents controlled by artificial intelligence. With each round, the difficulty increases. The final battle is a battle with all enemies, despite the fact that the health bar does not increase when moving from one enemy to another. In total, the player has a choice of 7 fighters with unique movements and abilities. To defeat the enemy, you need to empty his health bar. If the Game Gore setting is enabled (on some consoles), when the health bar is empty, the heart next to it explodes.

During the battle, people run around the battlefield and worship their God. These people can be knocked up with their blows or eaten to restore some health. During the final battle, you need to eat as many people as possible in order to restore health.

Unlike most fighting games, where special moves are performed by first moving the joystick in a certain direction, and after pressing one or more keys, in Primal Rage you must first press the button located below, and then move the joystick. In the later versions after the arcade, performing "special moves" is done in a more traditional way, but in the settings you can set the keys for special moves. After defeating an opponent, the player is given time to perform a fatality. In addition to fatalities, there are other ways to finish off: for example, Vertigo can turn the enemy into a cow, and the inscription "La Vache Qui Rit" will be displayed.
